How do I start services?
Complete the enrollment form and submit. Once we receive your form, a staff member will reach out to you to answer your questions, gather more information, and complete the enrollment process.
Submit required documents. You must provide a prescription for applied behavior analysis (ABA) from a medical doctor along with a diagnosis for the individual and a copy of the front and back of your insurance card(s).
Once all documents have been received, benefits will be verified. Your insurance provider could take up to 14 days to approve the services.
